xj 24381d6272 test(hooks): add integration test for full hook pipeline 
Cover void hooks (gateway start, after tool call), modifying hooks
(before_tool_call pass-through and cancellation), and priority-based
dispatch via a CounterHook and ToolBlocker test pair.
